Our practitioners offer: Chiropractic assessment and treatment, Registered Massage Therapy, Active Release Techniques®, Graston Therapy, Medical Acupuncture, and Rehabilitative Exercise.
What Should You Do Immediately Following an accident?
Seek care from your primary health care practitioner (Chiropractor or Medical Doctor) for the proper assessment of your injuries. *Please note that it is NOT necessary to receive a medical doctor referral to receive care at Burlington Sports and Spine following your accident.
File an injury accident report with the police. Remember to keep a copy for your records.
Complete the Notice of Loss and Proof of Claim Form. Make sure you keep a copy for your records and send the signed copy to your insurance company within the following time frames:
For sprains, strains, or whiplash I or II, you must submit this form within 10 days of the accident to access the “Diagnostic and Treatment Protocols” coverage. If you have been diagnosed in this category you will have 90 days to access up to 21 pre-approved visits from your chiropractor.
For other injuries or if you choose not to access the “Diagnostic and Treatment Protocols” coverage you must submit this form within 30 days of the accident.
After this form is submitted your insurance company will contact you.
What Happens If You Are Still Injured After 90 Days?
Patients who wish to access the “Diagnostic and Treatment Protocols” coverage after an accident will have 90 days to receive 21 pre-approved visits from their chiropractor at Burlington Sports and Spine. After this 90 day period, if still injured, the patient will enter into the post-protocol phase of their treatment where they will need to pay out of pocket and bill their extended health benefits for treatment received. If this coverage is ever exhausted or if the patient does not have extended health benefits then they will enter into the “Section B” portion of their policy which will allow them access up to $750 for chiropractic, $600 for physiotherapy, $250 for Registered Massage Therapy, and $250 for acupuncture.Book an Appointment
